To overcome these drawbacks, selective electro-thermolysis, in which electrical energy from radiofrequency (RF) current selectively heats target tissue without damaging the epidermis, has been introduced. RF current placed on the skin penetrates the epidermis and moves to tissues that offer high electrical conductivity. Since conductivity varies inversely with temperature, cooling the epidermis guides the RF current to deeper tissues which, when pre-heated, has higher conductivity and thus a greater probability of receiving the RF current. The amount of heat generated in the pre-heated tissue varies with the tissue’s resistance (impedance) to the RF current.

Today a Variety of Options are Available for the Management of Pterygium

Pterygium usually refers to a benign growth of the conjunctiva. A pterygium commonly grows from the nasal side of the sclera. It is associated with, and thought to be caused by ultraviolet-light exposure (e.g. sunlight), low humidity, and dust. The predominance of pterygia on the nasal side is possibly a result of the sun's rays passing laterally through the cornea where it undergoes refraction and becomes focused on the limbic area. Sunlight passes unobstructed from the lateral side of the eye, focusing on the medial limbus after passing through the cornea. On the contralateral side, however, the shadow of the nose medially reduces the intensity of sunlight focused on the lateral/temporal limbus.

Today a variety of options are available for the management of Pterygium, from β-irradiation, to conjunctival auto-grafting or amniotic membrane transplantation, along with glue and suture application. As it is a benign growth, Pterygium typically does not require surgery unless it grows to such an extent that it covers the pupil, obstructing vision or presents with acute symptoms. Some of the irritating symptoms can be addressed with artificial tears. However, no reliable medical treatment exists to reduce or even prevent pterygium progression. Definitive treatment is achieved only by surgical removal. Long-term follow up is required as pterygium may recur even after complete surgical correction.

If there is recurrence after surgery or if recurrence of pterygium is thought to be vision threatening, it is possible to use strontium (90Sr) plaque therapy. 90Sr is a radioactive substance that produces beta particles which penetrate a very short distance into the cornea at the site of the operation. It suppresses the regrowth of blood vessels that occur with return of the pterygium. The treatment requires some local anaesthetic in the eye and is best done at the time of, or on the same day as the pterygium excision.

Facial Implants Are Used to Improve Balance In Facial Features

Facial implants are used to improve balance in facial features. The implants are specifically designed for cheek and chin augmentation. There are many types and shapes of implants available from different manufacturers using a variety of materials. Materials commonly used are Silastic and Gortex.

Implantation of a chin implant takes place through an incision approximately 3/4 inch long on the under side of the chin. A pocket will be created over the bone for the implant. The appropriate sized chin implant is then placed inside the pocket. Once the implant is in position, the skin will be closed with sutures. Typically there is some bruising and swelling around the chin area for a week to ten days following this procedure.

Implantation of cheek implants takes place through an incision inside the mouth just above the front teeth, creating a pocket, under the upper lip. The appropriate sized cheek implants are then placed inside each pocket. Once they are positioned, the skin will be closed with dissolvable sutures, which will fall out in five to six days. After the cheek implants are in place, it is rare to have them migrate or move. This operation includes liposuction of the neck, manual removal of fat underneath the muscles and tightening of the neck muscles under the chin. Excess skin cannot be removed with this procedure. If excess skin is the problem then a full face-neck lift should be done.

A one inch incision is placed just under the chin, and the skin is elevated off of the fat and muscle. A 3mm cushion of fat is left on the skin, and then liposuction is used to remove all of the remaining fat down to the muscle. De-fatting the neck is also done by manually removing fat from underneath the platysma muscle. Once the fat is removed, the muscles are tightened and any neck cords are released.

The incision is then closed with 3-4 sutures. Drains are also used to remove any excess fluid or blood and are removed in 1 or 2 days after the surgery.

The typical stages of breast reconstruction are:
Stage 1. Insertion of Tissue Expander

Over time, sterile saline fluid is added in order to slowly stretch the skin, so that it will eventually fit an implant.2, 3
Stage 2. Insertion of Implant
After the expander is removed, your surgeon will insert a breast implant. Many Surgeons offer a variety of silicone gel-filled or saline-filled breast implants that complement the existing expander.
Stage 3. Nipple and Areola Reconstruction
Reconstruction of the nipple and areola is usually performed as a separate surgery.
